Although it is a toxin, it is safe when administered by a trained medical professional. The most common minor complication is a slight and very temporary drooping of the eyelid. Is the procedure painful?
A very tiny micro-needle is used to inject the BOTOX Cosmetic, with a small amount injected at various sites on the face. A topical anesthetic is applied to the treated area prior to the injections to minimize any discomfort.

Results are usually seen within 4-7 days.
How long do results from BOTOX Cosmetic last?

So although the effects are not permanent, there may be a gradual lengthening of the interval between treatments.
